Course Extension Request Form

STUDENTS NEED TO COMPLETE AND SUBMIT THE FORM BELOW TO REQUEST AN EXTENSION TO YOUR COURSE (ONE FORM PER COURSE).

The purpose of this form is for students to request an extension for a course beyond the semester end date. It is therefore not for extensions on individual assignments, which can be granted by your professor on a more informal basis by way of a simple conversation. It is recommended that you have a conversation with your professor about the course extension that you're requesting before completing and submitting this form.

NOTE: If you are requesting a course extension for more than one course, you will need to submit a separate form for each course.

5. WHAT EXTENSION DATE ARE YOU REQUESTING BY WHICH ALL ASSIGNMENTS WILL BE SUBMITTED?

NOTE: For courses in the fall semester, students are not permitted to request an extension date after April 15. For courses in the spring semester, students are not permitted to request an extension date after August 15. And for courses in the summer semester, students are not permitted to request an extension date after December 15.

Before you submit your final assignments for this course and they are graded, you will receive an 'I' (Incomplete) on your transcript in the interim. This will be changed to your final grade upon the submission and grading of all your assignments after the requested date of your course extension.

I UNDERSTAND THAT IF I FAIL TO COMPLETE THE COURSE BY THE EXTENSION DATE THAT I HAVE REQUESTED ON THIS FORM, I WILL RECEIVE A FINAL LETTER GRADE ON MY TRANSCRIPT BASED ON THE CUMULATIVE PERCENTAGES FOR THE ASSIGNMENTS THAT I HAVE SUBMITTED UP TO THE REQUESTED COURSE EXTENSION DATE, INCLUDING ZEROS ON MISSING ASSIGNMENTS.
